It tells you exactly how to get the file in the Package Manager grid without doing a regular download at all, without going to GitHub, and without getting the messed up file you got.
Here’s what I think happened in case you’re curious. I suspect that you got the file from GitHub. The download screen there is confusing. To get the Transport Package file, you click on the filename. You most likely clicked on the "Download .zip file" button instead, which gets you a zipped version of all the files I use to build the package (not what you want at all).

It seems to me that forwarding users has been quirky in some browsers in the last few versions of MODx. I’ve seen it in various add-ons and in forwarding to the error and unauthorized pages. Sadly, I have no idea why it’s happening or how to reproduce it reliably.

SPform is not really designed for that. It’s meant to be a quick and easy contact form. You would have to rewrite the spformproc class to process the extra field.
I think you’d be better off switching to FormIt, which is harder to set up initially but is infinitely more flexible and much easier to customize.
Is any one else having trouble downloading this from the Package Management in the mangers panel in 2.0.8-pl Rev? Nothing happens when I click download. All other extras download fine but not this one. Also, on another site I have it installed but says there is an upgrade available, but it does not allow me is download it.
