I do. Switching to Firebug’s net panel (easier to read), I can see all the params and the full response, and I think I either see the culprit or modx + extjs is even more mind boggling complex than I already find it. The TV in question is #23 and is comprised of 3 separate TV, as you instructed in your example. So maybe this is deliberate tv2300000001, tv2300000002, tv2300000003? It’s the only thing I can find, besides Ext itself, that looks strange.
The first part of the response includes a hidden input:
<input type="hidden" class="mulititems_grid_item_fields" name="mulititems_grid_item_fields" value='[{"field":"title","caption":"Tab Title","tv_id":2300000001,"array_tv_id":"2300000001[]"},{"field":"description","caption":"Tab Pane Content","inputTV":"richtext","tv_id":2300000002,"array_tv_id":"2300000002[]"},{"field":"image","caption":"Image","inputTV":"image","tv_id":2300000003,"array_tv_id":"2300000003[]"}]' />
You also pass some sort of configuration object to a load method with the same value:
var fld2300000003 = MODx.load({
xtype: 'modx-panel-tv-image'
,renderTo: 'tv-image-2300000003'
,tv: '2300000003'
,value: ''
,width: '97%'
,listeners: {ETC}
});
Here are the parameters:

action mgr/fields
class_key modDocument
itemid 0
record_json {"title":"Tab One","description":"<h2>Tab Content</h2>\n<p>Lorem ipsum dolor sit amet, ...ETC...</p>","image":"assets/resources/images/placeholders/dummyimage.png"}
tv_id 23
