Like most things MODX, there are different ways to do things. In the default ht.access file, these are the uncommented lines that do the work:
# The Friendly URLs part
RewriteCond %{REQUEST_FILENAME} !-f
RewriteCond %{REQUEST_FILENAME} !-d
RewriteRule ^(.*)$ index.php?q=$1 [L,QSA]
I've updated the docs and a related page.
I hope they are clearer now.
