I first noticed this problem when I did a fresh Ubuntu 13.10 install on a Digital Ocean droplet. I have never had this problem after years of setting up MODx sites. But when I ran the setup everything ran fine but it wouldn't let me log in. So checking the mod_users table it had a blank user. Then if I reran the setup I get a database connection failed. The whole time I never touched a thing and the config file has all the correct information the the original setup.

I tried this on my DO droplet a few times and have tried a number of times with 2.2.11, 2.2.12, and 2.2.13 on a local vagrant. I originally noticed the problem when doing an upgrade and moving to a new server. Did I miss a server setting somewhere? But it would confuse me that everything else would write to the database correctly. Just wondering if anyone has seen anything like this before. It appears disabling opcache made everything run smoothly.

This post helped me figure it out.
