Are there any permissions in your resource policy? In the past, the media resource policies have been blank for some installations, admin policy in particular. Check the policy templates, if they're blank that very well could be the problem.

I think there should be 4 policies in the std media source user template, and 7 in the Admin. I know what all 7 are, but all my sites now have empty admin media source templates again.
I just checked five 2.2.8 sites. All empty for media source admin, and user is now back to 7 policies. I've been through this already and had reset at least 3 of those sites (2 are new dev cloud) templates with the correct policies back in 2.2.6, I believe. Now back to missing policies. Not good.
Right, so you can use all 7 that are shown in the user template for the admin template. Either duplicate the user and rename it as admin, or select them by hand. I am not 100% sure which is the 4th for the user template but load, list, and view are the first three. I am not sure if copy or save should be the other one or not. Might not be either of those as well.
This is frustrating.
You might take a look for them in the DB in the modx_access_policies table. It's very unlikely that they would be deleted. More likely, something has happened to make them invalid like an invalid template (look for the template ID in the modx_access_policy_templates table), or that the part of the cache containing them has been corrupted.
Alright folks, this is sort of anti-climactic and not very helpful. But I moved off a SUSE Linux VM to a CentOS VM, re-installed MODx, imported the database tables regarding content, htmlsnippets, templates etc. Also moved the physical template files over and it resolved the issue. I just have to revise the .htaccess as subpages don't work, I am sure I will get that running by the end of today if I don't get distracted.
