Formz installation causes site to go blank.

Upon further troubleshooting, the site goes blank when a resource uses a template w/ the form TV, but does not have a form on that page. I created a separate template with the form TV for the pages that use forms and as along as a form is selected, the site does not go blank.

I had a similar issue with Formz on the Cloud.
My site had the white screen of death on both front and back end. Cloud support confirmed it was the Formz snippet causing the issue.
