Glad you got it working! Sorry I wasn't of much help, but it was hard to identify a conflict with another plugin. I'll see if I can reproduce it and write a workaround for the next release.
Also, Bob, I didn't think anything of it when you first said Login was a dependency, but the more I think about it, it isn't -- you just need a form with the right input names. Unless that's something you changed in your additions that I didn't notice?

Is it possible to make it work on multi-contexts site?
How declare the ID of the form page, in the plugin, for each contexts?
Untested, but do you have the "allow_forward_across_contexts" system setting set to Yes? That should work. The form page uses the sendForward API call, so you should be able to use a single form page across multiple contexts if you enable that setting.
