Ok, so after some more testing it looks like it might be to do with the IIS server. I copied the whole modx site on to a LAMP 5.0 server without making any changes to code or settings, and the QE bar shows up straight away. So if anyone can think of what might be holding up the QE bar that might be to do with a Windows server...
