I'm experiencing the same problem with Evo 1.0.15 and KCFinder.
My PHP: 5.2.17
safe_mode is off
Larger files - i can upload, but with small I get this message on request to upload: "400 Bad Request"
Can you provide us with information regarding your hosting environment same as before? Also you should really upgrade your PHP version, as 5.2.x has been end of life (aka no support/security/bug fixes) for several years.

I've updated my PHP version to 5.3.29 and the problem still remains.
I is a shared linux hosting at www.home.pl, safe_mode - off.
Not sure what else can be important to share here.
What version of MODX do you have installed, where are you hosting it, what web & database servers and their versions?
