I hit a couple of snags.
First:
I now realize that the formit hook calls the snippet, and that I don't have to put the snippet call elsewhere on the page. Doing so causes the Internal Server Error.
Second:
The form worked for US, but not United Kingdom 'UK'. Changing 'UK' to 'GB' fixed that.
