Upgrade to 1.0.15 and white screen

Hi guys, testing upgrading a site today from 10.0.8 to 10.0.15 and following a successful upgrade install procedure - I now see a white screen for both the front end and manager... looking at firebug I have a 500 server error... any ideas on how to resolve???

I would think the config.inc file is blank.

Config file in /manager/includes is fine - full of correct data...?

Any way of diagnosing the issues?

ok, located error log in FTP - got this:

PHP Fatal error: require(): Failed opening required '/assets/plugins/forgotmanagerlogin/plugin.forgotmanagerlogin.php' (include_path='.:/manager/includes/') in /manager/includes/document.parser.class.inc.php(964) : eval()'d code on line 1

So if you upload the folder and file it works?
/assets/plugins/forgotmanagerlogin/plugin.forgotmanagerlogin.php

I think I had this problem aswell some time ago, deleted the folder but not the snippet in the manager so the login /manager didn't work properly.
