You’ve got a problem with session handling on your server. PHP expects to store session files in the /tmp directory, but it needs to exist and be writable so PHP can write the session files there.
so is it to do with permissions not being correct on 1 of my folders?

Possibly. It is also possible that your server doesn’t have a /tmp folder; many times it’s actually /something/tmp, with a symlink to make it act like it’s in /tmp. This is a server issue causing PHP to not be able to save its session files, however, and has nothing to do with MODx.
You mentioned that you have MODx installed in a directory "content". Wouldn’t you need to access it with
http://www.mydomain.com/content/install/index.php? Or was that mydomain.com/manage url just a typo?
