Formz - Submission fields displaying in random order?

My Formz form's inputs appear in the correct order when the form is viewed on a page:

Name:
Email:
Address:
Color:

But when the form is submitted and then saved to the database (or emailed) the inputs appear randomly ordered:

Email:
Color:
Address:
Name:

The order appears different with each submission, apparently random. There are other Formz on this site behaving normally. Any ideas?
