How to create a custom form template output as Formit + Send to mail

If I understand you correctly you want to send a custom email when someone completes the form ?

if so you need to specify the emailTpl, see http://rtfm.modx.com/extras/revo/formit/formit.hooks/formit.hooks.email for more details.
