OK, problem solved.

Turns out FormIt is working just fine. So I'm little embarrassed to say it was my mistake. Typically I put the FormIt call in a chunk and put the chunk into my template. I had more than one chunk for various types for forms needed on the site. For some reason I placed one of the FormIt calls directly in the template (which I don't usually do. . . ). So while I was trying to figure out what was wrong with my FormIt parameters I wasn't actually changing the working form.
