Can't see "add item" contents within manager using MIGX

Version: MIGX 2.6.8, MODX Revolution 2.2.14-pl (traditional)

I have installed MIGX following instruction at http://rtfm.modx.com/extras/revo/migx/migx.backend-usage but when trying to access the "add item" panel within the manager to select an image but I am unable to any contents within the pop-up?

There seems to be a flash of graphics within the MIGX pop-up but then it goes completely white. [ed. note: SandersDesign last edited this post 12 years, 5 months ago.]

try either pointing to a TV

    {"field":"image","caption":"Image","inputTV":"GalleryImage"}

or without any defined TVs, just by defining the inputTVtype

]},
{"caption":"Image", "fields":[
    {"field":"image","caption":"Image","inputTVtype":"image"}
