How to format a date TV?

I created the TV DateEvent and called it as follows:

which outputs

21-05-2014 15:23:00.

I would like to change this so that it reads May 21, 2014 15:00:00.

Can anyone advise? I've spent hours using various combinations of PHP snippets without success.

If you look at the image of the time template variable in the post i've linked to above, you can see a slot for the "Date Format".
