Hi, I'm trying to create something where a visitor on the website inputs his zip code, and when he submits it returns specific names that correlate to the zip code.
Is there any add-on that could help me implement this?
If anyone could help me or point me in the right direction it would be greatly appreciated.

You could create a simple CMP where you can add/edit items into a custom-db-table.
