The fact that the version number doesn't change in the Manager suggests that some of the older files may not have been overwritten or may be corrupt or missing. That's fairly common if you use FTP to transfer the files individually.

I'm not sure if duplicate URIs could cause a problem, but SiteCheck will report them for you, along with permission issues, bad paths, invalid intersects, and a lot of other hidden problems that might interfere with an upgrade. http://bobsguides.com/sitecheck-tutorial.html

Thanks Bob, I'll give sitecheck a go.

      Edit:
I tried SiteCheck, the only problems found are a bunch of resources with empty aliases, and a few resources with duplicate pagetitles.
Not sure if this has anything to do with a failed upgrade.
I think I need to revert back, and turn furl off before trying another upgrade (forgot to do that before).

It looks like a file and/or directory permission issue. If you originally uploaded files via FTP as one user, but you're running the upgrade as a different (apache or php) user then this would happen.

I double checked all file permissions and folder/file ownerships but I can't find anything wrong.
Any idea what this parts of the log means?

It may just be letting you know that those fields already exist from an earlier run of Setup.

You can check in the database with PhpMyAdmin to see if the indexes are there. Even if they're not, it might slow things down slightly, but I don't think it will cause any serious problems.
