Sorry if this is blindingly obvious, but I am trying to change the default template, but the setting is missing entirely from the system settings page. Has it been re-named? Removed?

It should be there. Running setup again may restore it, but first take a look at the modx_system_settings table in PhpMyAdmin. Maybe it's still there but doesn't show for some reason. If so, make sure the value is the ID of an existing template.
It's not in the database either.

As long as the site already exists, setup won't ever touch any of your content or settings. Just make sure that the "Upgrade Install" checkbox is ticked (it should be automatically).
