You can't. You have to add it yourself.
If you take a look at the file (manager/controllers/default/header.php) you see at line 94 the placeholder for the username is set. You can add there a custom placeholder if you want something like :
$this->setPlaceholder('fullname',$profile->get('fullname'));
Yeah i see that the $profile not is the user profile.

What do you want exactly? Adding an image to a user?
There are 2 options for that.
1. A package called modAvatar, that adds a image field into the manager.
2. You can use Gravatar.
