Maybe I'm missing something, but it looks like the HeaderImage TV is missing the 'assets/' prefix. IOW, it just has the wrong value for some resources. Make sure the default value for the TV starts with assets/. Clear the site cache and then check the TV's value on any pages that still have a problem.

As Bob suggested the problem was in the basePath/baseUrl for the Media Source -> filesystem.
It's not immediately apparent how to check the "filesystem" configuration but I found it in the database by comparing the original db and the new db via phpMyAdmin. I simply copied the value from the original db to the test db and voila! (Since then I learned that right-clicking "filesystem" will expose the properties for editing. Tools->Media Sources->filesystem (right-click)->Update Media Source)
Interestingly, this value was in the original sql dump which I imported to the test db. How did it disappear? Is it possible this value was changed/dropped when I re-ran the site set up?
