In my form I have 4 radio button like

I am using formit for this form. How can I code this area in my form.

Hi Donquicky,
In my form I have radio button as per my first post, let assume there are 4 radio button. If the user select last radio button option then there is a textbox to be filled by the user. And other 3 option are normal. If how I will write the formit code so that it can handle this functionility.
