The master template has a [[$Header]] chunk (contains nav bar, etc) and the contact form chunk is called as:

to create a popup/modal form on every page.
The Contact Us page resource also uses the form chunk to include a non modal version of the contact form:

</div>
It's on this page that the issue occurs, when idSuffix is
null
