basically how would you go about creating an extra that doesn't use the popup window?
for example, when you go add a new Article, the URL changes and there is the full screen of form fields etc, not just the popup window.

I'm assuming that you mean a Custom Manager Page (CMP) extra with a menu entry in the Manager's Top Menu (under Components).
When you create the action (in System -> Actions), you specify an index.php file that picking the menu choice will execute. Whatever is returned from that file will be displayed in the right panel. It can be plain HTML if you like. It's usually ExtJS JavaScript with controllers, processors, and connectors, but it's not a requirement to do it that way.
How you structure it depends on what you need to do in your extra. There's an example of a simple HTML-based CMP in my book:
