I added the "id" column and it worked! Thanks so much!

I had an issue where I couldn't see the Formz Manager anymore as well but when I reinstalled it, I got an error using the wrong cloud ID (it was from the original cloud that I had cloned). In the error log I had this:

[2013-03-27 08:19:13] (ERROR @ /connectors/system/errorlog.php) Path specified for package translations is not a valid or accessible directory: /paas/cXXWrongID/www/core/components/translations/model/

It was stuck on a path from a previous cloud I had cloned.

I had to edit the path and input the correct Cloud ID in the Extension Packages system setting and it all came back and worked again.

Paths are normally stored using [[++core_path]] system settings, you should only need to clear cache for this to had worked. But either way I am happy you have it working now.

Yes, in this case clearing the cache didn't work.
