I installed Formz (also FormIt and FormItFastPack) on a 2.2.5 site in MODX Cloud. I followed the instructions - created the form, created the TV and added the TV, [[*form]], to the content area in my resource. The page is blank, so I am wondering if I am missing something REALLY simple.

    Thanks,

Check your error logs; not only the one in the Manager but also the one two levels up from the web root (use SFTP or SSH to view these logs).

That should be all you need. Did you attach the TV to the template the resource is using?

          Manager log shows the following:

          [2013-03-19 16:33:35] (ERROR @ /index.php) Error 42S22 executing query: SELECT `fmzForms`.`id` AS `fmzForms_id`, `fmzForms`.`name` AS `fmzForms_name`, `fmzForms`.`method` AS `fmzForms_method`, `fmzForms`.`email` AS `fmzForms_email`, `fmzForms`.`success_message` AS `fmzForms_success_message`, `fmzForms`.`identifier` AS `fmzForms_identifier`, `fmzForms`.`action_button` AS `fmzForms_action_button`, `fmzForms`.`createdon` AS `fmzForms_createdon`, `fmzForms`.`createdby` AS `fmzForms_createdby`, `fmzForms`.`editedon` AS `fmzForms_editedon`, `fmzForms`.`editedby` AS `fmzForms_editedby`, `Fields`.`id` AS `Fields_id`, `Fields`.`form_id` AS `Fields_form_id`, `Fields`.`type` AS `Fields_type`, `Fields`.`settings` AS `Fields_settings`, `Fields`.`order` AS `Fields_order`, `Validation`.`id` AS `Validation_id`, `Validation`.`field_id` AS `Validation_field_id`, `Validation`.`type` AS `Validation_type`, `Validation`.`error_message` AS `Validation_error_message` FROM `modx_formz_forms` AS `fmzForms` LEFT JOIN `modx_formz_forms_fields` `Fields` ON `fmzForms`.`id` = `Fields`.`form_id` LEFT JOIN `modx_formz_forms_validation` `Validation` ON `Fields`.`id` = `Validation`.`field_id` WHERE `fmzForms`.`id` = ? )

I noticed that when I clicked the "required" and choose a dropdown for the "validation" and then hit Save, the required column for the field says "No". Not sure why it won't save it as "Yes".

Can you check if this table was created modx_formz_forms_validation, I had some issues with the build script before which I think may be the cause of this. Also check if that table does exist if it has a id column.

Yes, because I can select the form that I created.

The table does exist and it has a field_id column. I am attaching a screenshot.

Thanks for that, its missing a id field in that table. I think the build script code didn't execute properly, try re-installing and see if the table gets added in.

I don't see a field named "id"; I do see one named "field_id".

I reinstalled formz and it is the same. Can I add "id" column?
