Your FormIt call seems to be missing a bunch of stuff. I use FormIt a lot just to send emails, my calls are usually like this:

Followed by an actual form. That link you posted suggests to me you need to include the hook 'formdata2db' as well as the others listed in my call and the fiProcessArrays does your validation.
Have you followed the rest of the instructions on that link regarding setting up additional tables in your DB, creating the formdata2db snippet, etc?
Edit: apologies, I mentioned redirect hook, but you already had that one, it was the others that were missing!
