Try this (untested)
1. set the checkboxes' names as the file paths,
2. then when it is submitted, use a custom hook to process the checkbox
3. in the custom hook, grabs the file(s) then apply the $_FILES variable from the files' $_POST name.
4. after that, let the email hook does its work.
