ModX 2.2.4 on a decent VPS.
Front-end of site: phpthumb/phpthumbof works great. I use it all over the place without issues.
Back-end of site: when viewing thumbnails in manager file browser or RTE browser, phpthumb spawns numerous processes that jack the server load to over 130+ (yes for real, 130), hangs the server and then my hosting provider suspends the account.
My images directory has several sub folders and at most has to load 50 thumbnails. What's odd, is that phpthumb re-processes the thumbnails every time and never seems to use the cached versions. I have tested this on another 2.2.4 site on a different host and it too processes thumbnails every time without loading from cache. The phpthumb cache folder has the thumbnails in there, just that the manager/rte browser never uses them and always re-processes.
Technical info below:
Gallery is NOT installed (I have read Gallery can cause some issues)
ModX 2.2.4 (all packages up to date)
VPS: Intel dual Xeon (8 cpu's), 1gb guaranteed ram

Well, since there were no responses, I just threw money at the problem and upgraded to a dedicated server with ridiculous horsepower. Site runs smooth now. Too bad it couldn't be solved the proper way.

I didn't catch your post earlier, but the usual cause of this problem is how the processors are handled with virtural machines. You may "have" 8 CPU cores, however when phpthumb comes into play they can quickly vanish. A Hybrid VPS server with 1 dedicated CPU core should be all you need, or going all out with a dedicated server.
I know we have done this for a few clients and it resolves the problem almost every time. I hope that helps a tad for anyone else having the same issue.
