Form Customization Breaking

I'm not quite sure what happened exactly, but my form customizations stopped applying to their form fields properly. Instead, now the default labels and visibility shows for create/resource and I now get what looks like an error right below the introtext field that says:

/usr/share/nginx/www/modx/core/components/gallery/elements/tv/output/

Any ideas what that might be?
