Add User Group, Policy field not loading

In Tools -> Media Sources I have a new Media Source and right click to Update Media Source and finally Add User Group. In this pop-up (Add User Group) the Policy drop down will load anything and therefore is broken.

The Policy drop down tries to load the first time, but then gives an error of "This field is required" and can not select anything at all. The policy.php file in connectors/security/access does load.

Any ideas about where to look in either code or the database to troubleshoot why this field isn't loading?
