Gallery upload same image always

Hello to everybody,

I have a problem with Gallery. When I upload an image despite it is shown that the images has been uploaded the new image is always the same with the very first image I have uploaded both in backend and frontend. Important notice: In the backend I can see that the 2nd uploaded image has the same name with the first one despite initially they have different name.
Any help would be highly appreciated because I encounter this problem in every site...
Thank you
