Assume someone with an existing site wants you to significantly add to their current site. How in the world do you do this effectively?

Setting up a local environment to manage development seems very hit or miss and there are all sorts of changes that seem to exist only in the database? Implemented via the backend interface only?

Clearly you can't work directly on a live site so how do you create a working local instance and then get those changes moved live? As far as I can tell you cannot really make use of version control (due to many changes being in the database only), if your local install does not have something like SSL enabled but the remote install uses the revoSSL plugin for the manager you are in trouble as you cannot access the manager at all, short of directly manipulating values in the database.
