I'm sorry to create so many threads on this issue but two weeks of googling this problem has me looking at a dozen different things. I'm also handicapped by not knowing how Manager works and ignorance about Apache, virtualization (cloud, load balancing?), and PHP.
From all my research I believe my problem is sessions/persistence issue while working in Revo Manager 2.2.1. Since my site was moved to new virtualization environment using OnApp and Xen hypervisor I've been unable to do much in manager without losing my login (permissions). Also, behavior is noticeably different among browsers. And I can't get to System Settings (or System anything) in any browser. I've modified .htaccess to use a local php save path, but to no avail.
This quote from opengeek leads me to question why the environment or php save path would matter since session data is stored in the database.

My hosting provider has referred this to OnApp but of course they want to know how MODx works before answering.
I am in the middle of going through BobRay's troubleshooting guide but ran into a snag not being able to clear out core/cache. I've asked my hosting provider to add my ftp user login to the httpd group, per info obtained from another forum.
