Apache 2.2.19
PHP 5.2.17
MySQL 5.0.92
I started a new site, created a template and had installed three extras. When I returned to work on it today, access to the Manager was fine, but immediately after installing CodeMirror, I couldn't access any part of the Manager and received a "Error! No action with [insert action number here] found."
I cleared the browsers caches (Safari, Firefox and Chrome) and deleted all the files in core/cache but still no luck.
I then deleted the MySQL database, created another one, and did a fresh install of MODX 2.1.3. The first thing I did after logging in was to head to Package Management and download and install Wayfinder. Clicking on anything after installing the package resulted in the same error message. Viewing the site was fine, but access to the Manager was no longer possible.
I have at least 10 sites on the same server, with various versions of Revo and have never encountered this problem. I then deleted everything in the public_html folder, deleted the MySQL database, downloaded a fresh copy of MODX and installed that. Same problem.
I'm now a bit worried to work on some of my other clients' sites as installing a new package may jam it up and screw up my deadlines. Any help would be appreciated.

I'm getting the same message and would like to know how to solve it.
Interestingly enough, the only thing I changed was to move my snippet out of modx and replaced it with a "require_once" statement. The page using the snippet still works but admin is giving me that error message.
Any thoughts?
terobot, did you clear the browser cookies as well. Sometimes a leftover cookie can cause weird stuff.

Frankly, I can't remember anymore. I can only assume I did. But at the time, I distinctly remember trying to access the Manager using different browsers on different computers and the problem occurred on each.
In the end I gave up and changed server - no problems since. I still have other MODX sites on the old server and have never encountered this problem either. Maybe I just had something dodgy in my config file.

I'm having the same problem after migrating my server and clearing caches by upgrade or delete files in core/cache.My configuration is IIS+sqlsrv.
Have tried deleting core/cache, browser cache & with different browsers, there is a modx_actions table with: 1 core 0 welcome 1 welcome, configcheck.
Any way to solve this? This is from version 2.1.2
