Thickbox should get the image title/description from the link title attribute. The template in maxigallery wiki has all in place, so you should see the titles and descriptions in thickbox by default. You don’t have to modify thickbox.js at all.

The title displays fine for the thumbnail alt-tag, but when the large image opens I don’t get either the title or the description underneath the image...

If PHx is not installed as a plugin, MaxiGallery will use it’s own PHx class to render the output, so no, you don’t need to have PHx installed.
