    Had this problem also - related to site_url in System Settings, make sure site_url contains trailing backslash: "http://www.mydomain.com/", not "http://www.mydomain.com"

      Hi, same exact problem as you the, formit not redirecting is a problem. I also friendly URL's and my .htaccess code is similar to yours. I do not see a site_url under system settings. I could have sworn I had seen it before. Using Revo 2.2.6. Any suggestions (mine worked for a few minutes and then back to not redirecting.

        My redirect issue was resolved. Under system settings and: "FURL Alia Strip Element Tags" set to yes. Now my form redirects properly to success page.
