I was able to add a phone number field with the help of another post. The phone number field works just fine (as far as I can tell). What I am worried about is the coding. Here is the coding I have for the eFeedbackForm chunk:

As you can see the Phone Number is what I added. I notice the coding is slightly different (I copied another post).
Will this coding I have work well, or is there going to be issues long term?
This works fine for my form.
