Any help here is VERY appreciated.
I have used eForm before with great success on Evolution v.1.0.2
The site I am currently working on is in Evolution v.1.0.4
I have done everything exactly the same and the form will not submit and validate - it just reloads.
Here is my code calling the eform snippet:
