The inclusion of name="vericode" is wiping out the default text, "Enter code on right", in the input area. When I remove name="vericode" the text appears but of course vericode verification is disabled.

Of course. The vericode itself takes up that value. You’ll need a label for the field with your prompt text.
It’s just an empty field for the vericode to go in. I’ve got an onclick event to remove the instruction text.
