FreeType needs to be enabled also.

If the direct link to vericode.php doesn’t produce an image, there’s almost certainly something wrong with either the GD/Freetype libs or their configuration on your server.
You could try replacing vericode.php in case it got corrupted in the transfer.
There’s also another, slightly different, vericode.php included in the SPForm package. But the odds are on a problem at the host.
