    Hi Everyone,

    Please help me. I need to add a listbox to my feedback form where user can select his/her title. This ’Mr/Mrs’ field should appear before ’Name’ field. I added simple ’select’ option. But my problem is that form report does not include ’Mr/Mrs’ field into the actual report. What should I do? Thanks a lot! Here is my code:

    <div class="fbForm">

    <p><span style="color:#900;">[+validationmessage+]</span></p>

    <form method="post" action="[~[*id*]~]" id="FeedBackForm">

    <div class="form">

    <select name="title" id="title">
    <option value="">Select title</option>
    <option value="Mr">Mr</option>
    <option value="Mrs">Mrs</option>
    </select>

    <div class="name">Full Name</div>
    <div class="error" id="fbNameError"></div>
    <div class="element"><input type="text" name="fbName" id="fbName" eform="???::1:Must be no less than 5 symbols.:#FUNCTION fbValidateName" value="[+fbName+]"></div>

    <div class="name">E-mail Address</div>
    <div class="error" id="fbEMailError"></div>
    <div class="element"><input type="text" name="email" id="fbEMail" eform="EMail:email:1:EMail &raquo; Wrong e-mail address." value="[+fbEMail+]"></div>

    <div class="name">Message</div>
    <div class="error" id="fbTextError"></div>
    <div class="element"><textarea name="fbText" id="fbText" eform="Message:html:1:Must be no less than 15 symbols.:#FUNCTION fbValidateText" cols="40" rows="5">[+fbText+]</textarea></div>

    <table cellpadding="0" cellspacing="0" class="captcha-test">
    <tr>
    <td>
    <div class="name">Spam Protection</div>
    <div class="captcha"><img src="[+verimageurl+]" alt="Verification Code" border="1"></div>
    </td>
    <td>
    <div class="name">Code on the picture:</div>
    <div class="element"><input type="text" name="vericode" id="vericode" value=""></div>
    </td>
    </tr>
    </table>

    <div class="submit"><input type="submit" name="fbSubmit" id="fbSubmit" value="Send"></div>

    </div>

    </form>

    </div>

      You need to add the eform attribute to the select tag.
      <select name="title" id="title" eform="Title::1">
