Send Yourself a Copy Checkbox

I’m creating a Send-to-Friend form and want to add a field checkbox “Send yourself a copy”.

My fields are:
Friends email:
Sender name:
Sender email:

I’d like to add a checkbox that can send a copy to the Sender’s email.
