    I am creating a form that will collect feedback/data from logged in users (the form is not accessible to non-registered users).
    Is there a way for the form to automatically collect the username/details of the member that is filling out the form and email them a copy as well as me on submit?
    or would they have to enter their username etc at the top of the form?

    Also is there a way to show/collect different data by selection of a radio button... ie: if the select ’Town’ the field shown is for Town Or if the select ’Postcode’ the field shown is for the Postcode?
    One solution i was contemplating is ’display’ options on CSS Divs and a bit of script... could that work?
