    hello,

I’m looking for some input/suggestions on how to achieve the following:

I need to create a form that consists of maybe about three pages - I need to be able to print out the form, sign it and the mail it by regular mail - so the form needs to look kind of nice. Is there an easy way to convert the form to a pdf file, print and save it? is there a way to save to form to the modx database?

    any ideas are very welcomed.

thank you in advance for your help.

Create a form in Acrobat Pro and make it downloadable. The bonus is that you can even have it submit the results to a URL which could then be incorporated into the database.

I thought of that as well but I’m thought that Acrobat Reader does not allow the user then to print or save the form?

With the latest version of Pro for authoring (and using the latest Reader) you can.

Acrobat is pricey if you are using legit software. There are a number of pdf form makers that work better and faster. I think you can do this with pdf995 as well as pdfMakerPro. Also I haven’t tried but I think that the PDFs generated by OpenOffice may be able to have live form fields but I have not checked that at all.

One other option would be to have the form be filled out on the website and then use a pdf converter to convert the form output to pdf.

Here is one example of a script that would do this: http://www.tufat.com/s_html2ps_html2pdf.htm

            Hope this helps.
