I am looking for a contact form with a specific layout that could be placed on my home page. On the form it will ask for the following:
~ 3-4 radio buttons to select a service "type"
~ Open text box to enter a name
~ Calendar box, when clicked on will also pull up a small calendar to select the date if wanted
~ Probably also a drop down box for a time of day
~ Email entry box
~ Name box (optional)
~ Phone number entry )optional
I know eForm can send email notifications and a notification to a mobile device, but I am wondering if there is a way to retrieve the information, say at the end of the day and maybe it’s 20 contacts, to import it directly in to a database program (Act Pro 2011 to be exact).
Maybe eForm will be easy to set up that way? Or, this might be the case I have someone develop this form me? Any thoughts?
