[Solved] Registration form not showing up

Can anyone help me with some custom fields in WebLoginPE?

The form just doesn’t show up, I added the appropriate labels and input tags for the fields but nothing is happening. The DB has the field names in it....I’m totally lost here. I’ll post whatever code you need any insight is much appreciated.
