Any document that is not assigned to a document group is open for everyone. And if that document group is not connected to a user group, it still won’t be restricted. As soon as a document is assigned to a document group that is connected to a user group, it can only be accessed by users belonging to that user group.
This applies to both manager users/groups (for manager access control) and web users/groups (for front-end viewing control), although manager user groups will not affect web users and web user groups will not affect manager users. The same document group can be connected to either or both, however.

That’s the best description I’ve seen of MODx access permissions. I hope you don’t mind, I adapted it for the Wiki and added some examples. As usual, it took a lot longer than I thought and I was pretty fried by the time I finished so please check over my examples when you have time and correct any errors you find. I’m sure there are some mistakes. Feel free to revamp the whole section.
