Jot form doesn't show after move to real server

Hi,

I’m developing a website in my local PC using XAMPP and after that I move it to hosting server. The strange thing is that Jot form doesn’t show at all after I move the website?

I played around little bit and found that if I disable cache make it form [! !] to [[ ]] it shows the form, but doesn`t check the form fields (security code, content, etc.).

So I decided that I need to be cached off like this [! !]. After that I found that if I remove &placeholders=`1` &output=`0` the form is showing, but I need to use this call for proper design order.

Here is the call:

Any ideas what could cause this very very strange situation?!

My XAMPP server types are:

Apache 2.2.11
MySQL 5.1.33
PHP 5.2.9

My Hostgator account are the same:

Apache 2.2.11
MySQL 5.1.33
PHP 5.2.9
